In this spine surgery lecture, principles of the surgical treatment of spinal metastatic disease are covered. Expeditious diagnosis and prompt surgery can maximise the chance of neurologic recovery. Patients with high-grade MESCC, secondary to radioresistant tumours may benefit from separation surgery followed by radiosurgery. SSI and failure of instrumentation are responsible for 2/3 of the complications.
